About Home Health at National Church Residences
At National Church Residences, Home Health is where our mission meets people in the place they most want to be: their home.
Our Home Health team brings skilled nursing, therapy, education, and support directly to patients. We help them recover from illness, injury, surgery, or hospitalization while staying safe and independent. Because we care for patients in their own environment, we see the whole picture: daily routines, caregiver support, home safety, and barriers that are easy to miss in a traditional healthcare setting.
Home Health is also one of the few settings where you work truly one-on-one with your patient. In a hospital, SNF, or IRF, you may be caring for a whole hallway. Here, your full attention goes to the person in front of you.
You'll work independently in the field, but never alone. Nurses, physical therapists, occupational therapists, speech-language pathologists, social workers, home health aides, clinical leaders, schedulers, intake specialists, and quality professionals all play a part in each patient's success, and they're behind you every step of the way.
Why therapists choose Home Health with us
- Focused, one-on-one patient care
- Clinical independence and room for critical thinking
- Variety: every patient, home, and day is different
- Meaningful relationships with patients and their caregivers
- Collaboration across many healthcare disciplines
- A real voice in improving how care is delivered
- The satisfaction of helping older adults stay healthy and independent at home
Room to grow
We promote from within, and many of our leaders started in the field:
- Our President of the Healthcare Division started as a physical therapist assistant.
- Our Director of Clinical Management started as a field physical therapist.
- Our VP of Senior Services started as a private duty nurse.
- Our Assistant Business Office Manager started as a private duty aide.
- Our Journey Guide Supervisor started as an aide.
At National Church Residences Home Health, you're not just completing visits. You're helping patients rebuild confidence, regain independence, avoid unnecessary hospital stays, and keep living in the place they call home.
Who we're looking for
We are seeking someone with a heart for seniors who wants to be part of a service-minded company that puts people first and makes a meaningful difference every day.
Make everyday wins possible. Help patients get back to their garden, walk safely to the mailbox, or keep up with the grandkids.
- Bring therapy home. Build treatment plans around real routines, familiar spaces, and what matters most to each patient.
- Put your clinical skills in motion. Assess mobility, build strength and balance, and help patients move with greater confidence.
- Be a coach and a cheerleader. Give patients and caregivers practical tools, and encouragement, to carry progress forward.
- Connect with a team that cares. Collaborate with nurses, therapists, and providers to support the whole person.
- Use technology with purpose. Track progress, coordinate care, and keep the team connected while working in the field.
A day in the role
- Turn home visits into everyday victories. Help patients safely navigate stairs, get out of a favorite chair, and walk with confidence.
- Create a plan with purpose. Assess strength, balance, and mobility, then tailor treatment to each patient's goals.
- Teach skills that stick. Guide exercises, practice safe movement, and show caregivers how to help between visits.
- Make the home part of the treatment. Identify fall hazards and recommend equipment or simple changes that support independence.
- Keep progress moving. Reassess what's working, adjust treatment, and collaborate with PTAs, nurses, and providers.
- Keep the care team connected. Use mobile technology to document visits, track progress, and share updates.
Give each patient your full attention. Provide one-on-one therapy that builds trust and focuses on their goals, at their pace.
See your care make a difference. Follow patients throughout their care journey, celebrating progress as they regain strength, confidence, and independence.
