About Circadia Health
Circadia Health is a growth-stage healthcare AI company on a mission to prevent avoidable hospitalizations and transform senior-care operations. Our Circadia Intelligence Platform combines:
- Contactless sensing that monitors respiration and motion with medical-grade accuracy
- Native predictive models that detect 85% of preventable adverse events several days in advance
- Enterprise integrations that operationalize predictions directly inside EHR, care-coordination, billing, and compliance workflows
Today, our technology touches 40,000+ post-acute patients daily across skilled-nursing, home-health, and home-care networks. We are backed by leading healthcare and AI investors and headquartered in El Segundo, CA.
About the Role
Transform bedside care with AI. As Circadia’s on-site NP, you’ll see patients first, act on predictive health signals in real time, and coach facility teams to turn data into earlier interventions—reducing preventable readmissions. You’ll blend direct patient care with tech-to-bedside integration through hands-on training, live demos, and shadowed assessments.
This is a full-time, in field role requiring Monday–Friday presence at our partner facilities in Carson City, NV.
What You’ll Do
- Direct patient care: Conduct initial and follow-up assessments on flagged residents; initiate/adjust plans of care; complete consent; coordinate with facility providers.
- Tech → bedside integration: Train and coach staff at the point of care, standardize use of Circadia into clinical workflows.
- Data into action: Interpret trend-based insights, prioritize high-risk patients, and drive timely interventions to prevent decline.
- Close the loop: Complete documentation/charts within the same day
- High-volume rounding: Efficiently complete visits across designated SNFs (typ. 30+/day where appropriate), focusing on new admits, facility-flagged patients, and Circadia alerts.
- Quality & product feedback: Surface field insights to improve workflows and tech integration in a clinical setting
- Mentor & collaborate: Support NP onboarding, share best practices, and partner with clinical ops/product teams.
- Regional flexibility: Cover sites within a 30–40-mile radius; for sites outside of that radius, mileage and lodging covered per policy.
How Success is Measured (KPIs)
- Visits: Target 30 patient visits/day
- Engagement: ≥80% Time to Acknowledgement Rate: <24h
- Documentation: Complete all notes using AI-powered tools within 24 hours of each encounter.
- Adoption & impact: Demonstrated workflow adherence at facilities; trend toward fewer falls/readmissions over time.
Must-Have Qualifications
- Certified and licensed Nurse Practitioner in NV; ANCC (or equivalent) in Adult/Family/Geriatric/Acute.
