Complex Care Services (CCS) is an interdisciplinary care model designed to support patients with multiple chronic conditions, frequent healthcare utilization, and complex care needs. Through proactive, relationship-based care, the CCS team partners with patients to improve health outcomes, enhance the patient experience, and reduce avoidable emergency department visits and hospitalizations.
