HomeMade, Mable, Leap in! and Mable Direct are entities under the Attain Healthtech Group of companies. While our companies are diverse, three similarities bind us:
1. We exist to create lasting, positive change across the entire care and support sector - impacting more lives, elevating standards and shaping the future of care
2. We believe in technology as an enabler
3. We’re driven by helping customers attain better outcomes
This position is part of the HomeMade team.
About HomeMade
HomeMade is an approved aged care provider dedicated to improving its customers' lives by empowering older people to self-manage their service providers with the assistance of our clinical and customer support teams.
Our transformative technology platform helps thousands of people retain choice and control by allowing them to create unique support plans that meet their specific needs and optimise their funds.
By working at HomeMade, you can play a crucial role in helping older people maintain their independence and live fulfilling lives in their own homes. You’ll be joining a team that is passionate and dedicated to making a meaningful difference in the lives of others.
About the role
As our Community Relationship Manager, you’ll be the face of HomeMade for our local community coordinators across Australia. You’ll be their go-to person, their champion, and their trusted partner. You’ll help them deliver exceptional service coordination to Home Care Package recipients, and you’ll help us grow a model that puts local people at the heart of aged care.
Why this role?
- Real impact - Your work directly shapes the quality of care older Australians receive.
- Variety - One week you’re running a workshop, the next you’re solving a complex case with our clinical team or supporting a business development meeting.
- A front-row seat to change - Help coordinators navigate the New Aged Care Act and shape how it’s brought to life.
- Connection - Build real relationships with passionate people in communities across the country.
- Room to grow - Work with senior leaders on strategic initiatives as we scale.
What your days might look like:
Building capacity and engagement
- Visit coordinators in person and connect virtually to understand what’s working, what’s hard, and how we can help
- Run workshops and reviews that turn policy into practical, confident everyday practice
- Design and deliver a consistent training program, including a streamlined onboarding experience for new coordinators
- Be the first point of contact for coordinators, making sure they know how to get what they need from HomeMade’s teams and feel at home using the HomeMade Portal
- Guide coordinators through the changes under the New Aged Care Act, from updated policies and training to rolling out changes to customers
Process improvement and growth
- Partner with coordinators and internal teams to spot and implement improvements
- Help translate strategy into practical procedures and documents, then train coordinators to put them into action
- Support our senior manager with business development initiatives, such as lead sourcing, stakeholder meetings, agreement drafting and sales generation
- Help scale our local community partner model
What you'll bring:
- A genuine passion for creating a bright future for older Australians
- An enthusiastic, can-do attitude and a hunger to learn and grow
- Proven experience in relationship management, with a knack for building trust and lasting partnerships
- Experience with home care recipients and funding in the aged care or community sector is preferable but not essential.
- Strong problem-solving skills and the ability to find practical solutions that balance the needs of customers, communities and the organisation
- Excellent communication and influencing skills that inspire confidence and drive positive change
- Great time management and the ability to juggle multiple priorities
- Experience planning, delivering and overseeing local community initiatives
- A valid Australian driver’s licence and a willingness to hit the road to regional and rural locations
Experience working with remote and regional communities is a bonus, but not essential.
