Our client based in Shropshire is seeking an experienced Senior Social Worker to join their Community Team, supporting adults across the North Shropshire locality. Working within Adult Social Care, you will play a key role in helping people remain safely within their own homes and communities, reducing reliance on long-term services wherever possible. You will work collaboratively with health professionals, carers, families, and partner agencies to achieve positive outcomes and support effective service delivery.

- Undertaking Mental Capacity Assessments and contributing to Best Interest decision-making in accordance with the Mental Capacity Act 2005.
- Working with individuals in hospital settings to facilitate appropriate, timely, and safe discharges back into the community.
- Coordinating services and support arrangements to prevent unnecessary hospital admissions and promote independent living.
- Working collaboratively with health colleagues, housing providers, carers, voluntary organisations, and other partner agencies to deliver integrated support.
- Identifying, assessing, and managing risk whilst balancing individual rights, choice, and autonomy.
- Promoting service user engagement and ensuring people, their families, and carers are involved in assessment, planning, and decision-making processes.
- Maintaining accurate and timely case records, assessments, reports, and support plans in line with legislative and organisational requirements.
