- Undertaking Mental Capacity Assessments and contributing to Best Interest decision-making in accordance with the Mental Capacity Act 2005.
- Completing safeguarding enquiries and interventions, ensuring adults are protected from abuse, neglect, self-neglect, and exploitation.
- Assessing and managing risk appropriately whilst supporting positive risk-taking and individual choice.
- Working collaboratively with health professionals, social care colleagues, carers, families, housing services, and partner agencies to deliver coordinated support.
- Promoting strengths-based practice and supporting individuals to maximise independence and improve their quality of life.
- Applying relevant legislation, professional standards, and social work values to support effective decision-making and practice.
- Participating in multidisciplinary meetings, safeguarding discussions, reviews, and case conferences.
- Maintaining accurate, timely, and legally compliant records, assessments, reports, and support plans.
- Contributing to service development, reflective practice, and continuous improvement initiatives.
