Social Worker- Specialty Outpatient Ambulatory Care Network - Columbia - Days

The Ambulatory Care Network provides services such as triage, screening, referral, and crisis intervention to ensure the mental well-being of our patients. You will be responsible for seeing patients virtually and in person, via scheduled appointments and walk ins. Position involves a wide range of responsibilities including but not limited to: crisis intervention, assessing for safety using evidence based screening, concrete services, assisting and following up with home care, psychosocial assessments, referrals to CBO's. Must work collaboratively with interdisciplinary team and across the network of ambulatory care clinics to cover Pulmonary, Cardiology, ENT/Audiology, Urology, Hepatology, and Dermatology for both adult and pediatric population.

The schedule for this full time position is Monday - Friday 9:00 AM - 5:00 PM.

Preferred Criteria

• Bi lingual Spanish

• Experience in a busy outpatient setting

• Experience working with interdisciplinary teams

Required Criteria

• Master's Degree in Social Work (MSW)
