Lead a team. Support a growing census. Make an impact close to home.
Elara Caring is seeking an experienced home health RN to join our Jackson team as a Clinical Team Manager. With our patient census growing, we’re looking for a leader who can keep the team supported, care coordinated, and quality at the center of every decision.
You’ll be a resource for our nurses and therapists—providing clinical guidance, helping navigate challenges, and supporting their professional development. Working closely with branch leadership, you’ll help ensure we have the staffing, communication, and clinical oversight needed to care for more patients in their homes.
Required: Current RN license, at least 2 years of skilled home health experience as an RN, and OASIS experience.
Why This Role Stands Out
- A growing census: Help guide clinical operations as we welcome more patients into care.
- Hands-on team leadership: Coach clinicians, answer questions, and help your team succeed.
- Meaningful clinical influence: Strengthen care coordination, documentation, and patient outcomes.
- Partnership with branch leadership: Have a voice in staffing, hiring, and clinical priorities.
- Continued connection to care: Support patients in the field when needed.
What You’ll Lead
- Oversee daily clinical operations, patient assignments, and coordination of home health services.
- Align staff assignments with patient needs as census grows.
- Coordinate referrals and support timely starts of care within regulatory guidelines.
- Ensure patient needs are continually assessed and individualized plans of care are developed, implemented, and updated appropriately.
- Review and approve care plans and proposed changes for clinical appropriateness.
- Conduct regular OASIS reviews to support accurate documentation and quality care.
- Coordinate physician orders and communication among clinicians, patients, families, and providers.
- Meet regularly with team members to provide coaching, address concerns, and communicate organizational updates.
- Participate in interviews and recommend direct care staff for hire.
- Communicate key performance metrics, staffing needs, and clinical priorities to the Branch Director or Administrator.
- Provide field support and complete additional duties or projects as needed.
What You Bring
- Current, unrestricted RN license valid in the state of Michigan.
- Associate degree in nursing or a related nursing field.
- At least 2 years of skilled home health experience as an RN, including OASIS experience.
- At least 1 year of supervisory experience preferred.
- Strong communication, organization, and clinical decision-making skills.
- Ability to become proficient in Homecare Homebase, Workday, and other enterprise applications; working knowledge of Microsoft Office.
- Willingness to travel throughout the branch coverage area.
- A valid driver’s license with an acceptable driving record, or reliable transportation between patient homes and assigned work locations.
- Ability to frequently lift, push, pull, and support up to 50 pounds, including assisting with patient positioning or transfers and moving equipment.
- Ability to manage the physical demands and competing priorities of a busy clinical environment.
