What will your responsibilities be?

- Undertaking Mental Capacity Assessments, applying the two-stage test, and facilitating Best Interest decision-making in accordance with the Mental Capacity Act 2005.
- Leading and completing safeguarding enquiries, including complex safeguarding concerns and whole-service safeguarding investigations where required.
- Assessing and managing risk effectively, balancing individual choice with safety and positive risk-taking.
- Working collaboratively with health professionals, multidisciplinary teams, community services, carers, families, and voluntary sector organisations to coordinate effective support arrangements.
- Supporting safe and timely discharges from hospital and ensuring robust follow-up support is in place within community settings.
- Undertaking work relating to the Court of Protection, including applications relating to deprivation of liberty in community settings where appropriate.
- Preparing high-quality assessments, reports, support plans, and professional documentation in accordance with statutory and organisational requirements.
